How do I ask for a name or an email address?

Use a Text field for a name, company or email address. For an email address, open the field's settings and set Validation to Email. Place the field right next to its printed label, such as "Email:", because the signer sees only "Type here..." in the signing card. See How the Text and Number fields work.
